Role Summary

• Provides direct patient care including assessment, monitoring, recording, and reporting patient condition changes
• Collaborates with the care team, participates in interdisciplinary care, and advocates for patients
• May need to step into an LPN Med Cart role as needed and when staffing permits
• Responsibilities include documentation in EMR

Required Qualifications

• Valid RN license in New York
• Associate of Science in Nursing / Associate Degree in Nursing
• Skilled Nursing (1 year of experience)
• Skilled Nursing experience within the last year
• BLS issued by American Heart Association or American Red Cross
• Driver's License
• Resume
• Two professional references

• MMR immunity or vaccination documentation
• Varicella immunity or vaccination documentation

• Centralized onboarding forms, acknowledgements, and competency/testing documents required by the employer
