Overview
- Full-time contract role in home health
- Day shift
- 36 hours per week
- Service area includes White River Junction, Lebanon, Bradford, and Windsor
Required Qualifications
- Bachelor’s degree in Physical Therapy
- Valid Physical Therapist license in Vermont
- 1 year of experience as a Physical Therapist
- Physical Therapist experience within the last year
- BLS
- Experience in a Home Health setting
Role Responsibilities
- Evaluate patients and develop treatment plans
- Carry out treatment programs according to agency policies and professional standards
- Communicate with the health care team
- Document evaluations, treatment goals, and objectives
- Teach patients and family members about treatments and procedures
Work Setting
- Home health
- Average visit length is 45 minutes
- Broad independence while working as part of a patient-care team
