POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Discharge Planning / Care Coordination / Continuum of Care / Patient Focus and Care Plans / Resource Utilization in an LTAC setting

MINIMUM REQUIRED QUALIFICATIONS – IL RN license; AHA BLS; 2 years recent RN Case Management experience, with experience in a LTAC or STACH setting, MCG, discharge planning across acute care settings, strong IDT collaboration

LENGTH OF ASSIGNMENT – 8 weeks

SHIFT / HOURS PER WEEK – M-F 8a-430p. Weekends on request.

START DATE – 6/8/2026
