RN Case Manager
StartDate: 6/8/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – In the acute rehab unit, coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS – MA RN license; BLS; 2+ years minimum of acute care hospital experience

PREFERRED QUALIFICATIONS – 2-3 years case management experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M-F, 40 hours 8a-4:30p. Weekends on request.
