RN Hospice Case Manager
StartDate: 6/15/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Hospice Case Manager

POSITION DUTIES – Coordinates and oversees all aspects of a patient’s end-of-life care - clinical, emotional, and logistical - to ensure comfort, safety, and support for both the patient and their family.

MINIMUM REQUIRED QUALIFICATIONS – 

• VT/compact RN license
• BLS
• 2+ years minimum of acute care hospital experience
• Experience with hospice case management 
• Experience providing hospice care in a home setting

PREFERRED QUALIFICATIONS – Prior experience with rural travel; McKesson/Netsmart EMR experience; No RTO requests for this contract

LENGTH OF ASSIGNMENT – 20 weeks

SHIFT / HOURS PER WEEK – Mon-Fri

SYSTEMS – McKesson/Netsmart

START DATE – ASAP
