StartDate: 6/8/2026 Pay Rate: $1600.00 - $2400.00
POSITION SUMMARY – Social Worker, LMSW
POSITION DUTIES – Complete SW assessments, SW Mental Health assessments, Title 36 petitions as needed, DC planning as needed, homelessness, substance abuse, SDOH consult, maternal child assessments, DCS and APS reporting as needed
MINIMUM REQUIRED QUALIFICATIONS –
LMSW or LCSW (AZ license required)1+ year recent experience in specialtyAdditional experience/knowledge: complete SW assessments, SW Mental Health assessments, Title 36 petitions as needed, DC planning as needed, homelessness, substance abuse, SDOH consult, maternal child assessments, DCS and APS reporting as neededLENGTH OF ASSIGNMENT – 15 Weeks
SHIFT / HOURS PER WEEK – Typically 0800-1630, weekends would be maybe every 3-4 weeks if at all (40hrs/wk)
SYSTEMS – Cerner
