RN Case Manager
StartDate: 7/13/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY – RN Case Manager

POSITION DUTIES – Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.

MINIMUM REQUIRED QUALIFICATIONS –

• FL/compact RN license
• BLS
• 2 years case management experience in an acute hospital setting
• Acute hospital experience during the past year with no more than a 90-day break
• DCP Assessment skills
• Experience with Meditech, Midas, InterQual

PREFERRED QUALIFICATIONS – CCM

LENGTH OF ASSIGNMENT – 12 weeks

SHIFT / HOURS PER WEEK – 9:00AM - 5:30PM

SYSTEMS – Meditech, Midas

START DATE – ASAP

Facility Location
Sanford is located in the central region of Florida and used to be home to various Indian tribes who inhabited the shores of Lake Monroe. Orange Groves and agriculture dominated Sanford’s economy until the early 1940’s when it was later deemed cheaper to cultivate in southern Florida as it was a frost-free zone.
