POSITION SUMMARY – LCSW Social Worker

POSITION DUTIES – Assess and drive treatment planning during acute admission and transition to discharge to assure the proper utilization of services at the most appropriate level of care for all patients, both inpatient and outpatient, who receive treatment through the Centers for Behavioral Health.

MINIMUM REQUIRED QUALIFICATIONS –

• Massachusetts LCSW
• BLS
• CPI
• 2 years social work experience in an acute hospital setting

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – 40 hrs, 8a-4p. rotating weekends (about 1x per month)
