POSITION SUMMARY – LSW Social Worker
POSITION DUTIES – Provides services to customers and their families/significant others, to help them adjust to illness, injury or hospitalization. Assists the health team in understanding the significance of social, economic, emotional and family factors in relation to the customer's illness, treatment, recovery or dying processes. Responsible for facilitating timely, efficient, and appropriate discharge of customers and coordination with next level of care, including transmission of appropriate plan of care. Identifies needed community services and coordinates appropriate services for customers in need.
MINIMUM REQUIRED QUALIFICATIONS –
- Minnesota LSW, LGSW, or LICSW.
- 2+ Years Hospital Social Working Experience
- Discharge Planning Experience
- All Patient Populations Experience
- Epic Experience
LENGTH OF ASSIGNMENT – 10 weeks
SHIFT / HOURS PER WEEK – 8:00 AM - 5:00 PM, with every 7th weekend.
