RN Case Manager
StartDate: 8/17/2026 Pay Rate: $1800.00 - $2700.00
POSITION SUMMARY – RN Case Manager
POSITION DUTIES – Works with physicians and multidisciplinary team members to develop a plan of care for assigned patients. Ensures patients are progressing towards desired outcomes by monitoring care through assessments and/or patient records. Identifies and resolves barriers that hinder effective patient care. Actively involved in discharge planning process.
MINIMUM REQUIRED QUALIFICATIONS –
- MT/compact RN license
- American Heart Association BLS
- Minimum of 2 years of nursing experience
- Utilization review experience
PREFERRED QUALIFICATIONS –
- Bachelor’s Degree in Nursing
- 2 years of acute care hospital experience as a case manager
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – 730am -430pm, Weekend Rotations
SYSTEMS – Epic
START DATE – ASAP
