POSITION SUMMARY – MSW Social Worker

POSITION DUTIES – Conducts psychosocial assessments, coordinates discharge planning and community resources, provides counseling and patient/family support, and collaborates with the interdisciplinary team to promote patients' functional recovery and safe transition to the next level of care.

MINIMUM REQUIRED QUALIFICATIONS – 

• MSW
• BLS
• 1+ year social work experience in an acute hospital setting

PREFERRED QUALIFICATIONS – Inpatient rehab / acute rehab experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Days with weekends on request

SYSTEMS – Epic

START DATE – ASAP
