POSITION DUTIES Home health visits; follow up on discharge 

MINIMUM REQUIRED QUALIFICATIONS Must have 1 year of Home Health in the last 3 years; CA license

PREFERRED QUALIFICATIONS Kaiser experience

LENGTH OF ASSIGNMENT 13 weeks

SHIFT / HOURS PER WEEK 0830-1700 Mon-Fri; no weekends
