StartDate: 9/14/2026 Pay Rate: $1800.00 - $2700.00

POSITION SUMMARY Montefiore Nyack Hospital offers the best of both worlds: the resources and advancement opportunities of a major health system combined with the welcoming culture of a close-knit community hospital, all located in the picturesque Hudson River village of Nyack with easy access to New York City.

POSITION DUTIES Case Management, Discharge plan, Navigation Plan, Education patients post-surgery, Case Management RN,Ortho Care Coordinator experience preferred; No Utilization review that is handled by another department Experience/familiar with Parachute and Careport software related to Ortho

MINIMUM REQUIRED QUALIFICATIONS NY License, BLS required, BSN NOT required, Minimum 2 Years Experience

PREFERRED QUALIFICATIONS ACLS preferred, Experience in managed care performing utilization review and discharge planning preferred, HC-PRI Certified and PRI screen preferred. CCM certification preferred,  ICU or ER experience preferred

LENGTH OF ASSIGNMENT 8 Weeks

SHIFT / HOURS PER WEEK 40 hours per week, Monday Through Friday, no weekends, 8-430, 830-5
