    $Hospital-Physical Therapist – Skilled Nursing Facility (SNF) 📍 Lawrence, Massachusetts 🏥 Mary Immaculate Health/Care Services 📅 13-Week Contract Assignment Schedule Day Shift 40 Guaranteed Hours Weekly No Call Requirement No Holiday Requirement Position Overview Mary Immaculate Health/Care Services is seeking an experienced Physical Therapist (PT) for a 13-week Skilled Nursing Facility (SNF) contract assignment. The Physical Therapist will work collaboratively with residents, families, and interdisciplinary healthcare teams to provide therapeutic services focused on maximizing functional independence, mobility, and quality of life. The ideal candidate will have prior Skilled Nursing Facility experience, strong clinical assessment skills, and the ability to develop individualized treatment plans for geriatric and rehabilitation patients. Responsibilities Perform comprehensive physical therapy evaluations and assessments Develop individualized treatment plans with measurable short- and long-term goals Provide therapeutic interventions to improve mobility, strength, balance, endurance, and functional independence Monitor patient progress and modify treatment plans as appropriate Educate residents and caregivers regarding therapy plans, exercises, safety techniques, and assistive devices Collaborate with physicians, nursing staff, and interdisciplinary care teams regarding patient care and discharge planning Recommend and train patients on appropriate assistive devices and mobility equipment Document evaluations, treatment sessions, patient progress, and discharge planning accurately within the EMR Ensure compliance with Medicare, Medicaid, and other reimbursement guidelines Maintain infection control and safety standards within the therapy department Supervise Physical Therapist Assistants and support staff as appropriate Participate in interdisciplinary team meetings and care conferences Requirements Bachelor’s Degree in Physical Therapy required Master’s or Doctorate in Physical Therapy preferred Active Massachusetts Physical Therapist License required Previous Skilled Nursing Facility (SNF) experience required Minimum 3 years of Physical Therapy experience preferred Strong documentation and time management skills Ability to work effectively within interdisciplinary healthcare teams/wk

    Position Overview
