📍 St. Johnsbury, VT | 🗓️ 13 Weeks | 🏡 Home Health Occupational Therapist

Caledonia Home Health Care & Hospice is seeking an experienced Occupational Therapist to provide in-home therapy services for home care and hospice patients throughout the community. This role is ideal for an independent, compassionate OT who enjoys working one-on-one with patients in a home health environment.

The Occupational Therapist will evaluate patient needs, develop individualized treatment plans, and provide therapeutic interventions focused on improving independence, safety, mobility, and quality of life. Candidates should be comfortable working autonomously while collaborating closely with an interdisciplinary care team.

This is a Monday–Friday assignment with up to 40 hours per week and no holiday or on-call requirements.

Requirements:

Active Vermont OT License
Valid driver’s license and reliable transportation
Home health or hospice experience preferred
Strong communication and documentation skills
Experience with Homecare Homebase (HCHB) preferred
