📍 New Lisbon, WI | 🗓️ 13 Weeks | 🏥 LTC RN – PM Shift

Crest View Nursing Home is seeking an experienced Registered Nurse for a Long-Term Care assignment on the PM shift. This role is ideal for an RN who is passionate about resident-centered care and enjoys working in a skilled nursing and long-term care environment.

The RN will assist with day-to-day nursing operations, including resident assessments, medication administration, wound care, condition monitoring, documentation, and communication with physicians and families. The nurse will also serve as a clinical leader on the floor, providing guidance and support to CNAs and other staff members.

Schedule:

PM Shift: 2:00 PM – 10:00 PM
Full-Time
Every Other Weekend
36 Guaranteed Hours
No Call

Requirements:

Active Wisconsin RN License
Current BLS/CPR Certification
Long-Term Care or Skilled Nursing experience preferred
Strong assessment, documentation, and leadership skills
Excellent attention to detail and communication abilities

Responsibilities:

Complete resident assessments and monitor changes in condition
Administer medications following the 5 Rights
Perform wound care and ongoing assessments
Communicate significant condition changes to physicians and families
Assist CNAs with resident care needs and call lights
Maintain accurate and timely documentation
Report concerns related to resident safety and well-being
