Description
- The RN – Case Manager provides nursing care and case management to patients in their homes.
- The RN assesses patient needs, coordinates care, manages treatment plans, educates patients and families, and works with other healthcare professionals to ensure quality care.
Key Responsibilities
- Provide skilled nursing care to patients at home.
- Assess patients’ physical and emotional health and monitor changes.
- Develop and follow patient care plans.
- Coordinate care with doctors, nurses, aides, CNAs, and other healthcare providers.
- Supervise Home Health Aides and CNAs.
- Educate patients and families about medications, treatments, and care.
- Maintain accurate patient records and OASIS documentation.
- Participate in team meetings, scheduling, on-call duties, and quality improvement programs.
Requirements
- Current RN license in Montana.
- BSN preferred.
- Valid driver’s license, auto insurance, and reliable transportation.
- 2+ years of recent acute care, home health, or hospice experience preferred.
- Experience with wound care, venipuncture, catheterization, or IV/parenteral therapy preferred.
- HCHB experience strongly preferred.
