Description
Qualifications include BS degree or better from an accredited Physical Therapy program with current license to practice in the State of Montana, valid driver’s license, reliable transportation, and appropriate auto insurance.
Two years of previous PT experience preferred.
Experince with Homecare Homebase HCHB
BLS required
Roles and Responsibilities include:
Provides direct patient care based on the established care plan and physician orders.
Masters scoring of the CMS required OASIS and works with Quality team to collaborate needed adjustments.
Practices point of care documentation in all direct patient care settings. Documents all skilled services provided to patients, adverse events, and relevant communications in the medical record.
Evaluates and establishes plan of care to meet patients’ needs in the home environment. Coordinates care with other disciplines and seeks orders for appropriate additional services.
