Minimum 2 years of prior experinece required
AHA BLS Certification required.
Job Description:
The Occupational Therapist is responsible for evaluation and rehabilitation intervention in the home setting, including management and supervision of patient care and patient and family education.
Shift: Day shift
Job Duties:
• Evaluates patients and establishes individualized home care plans, coordinating with other disciplines and obtaining orders for additional services as needed.
• Provides direct patient care according to care plans and physician orders, applying evidence-based occupational therapy practices, Medicare regulations, and Conditions of Participation in evaluations, 30-day reassessments, and treatments, adjusting care frequency and duration as needed.
• Completes CMS-required OASIS assessments accurately, collaborates with the Quality team on adjustments, and maintains a basic understanding of home health insurance requirements.
• Participates in multidisciplinary care conferences, communicates effectively with physicians and team members, and appropriately plans admissions, OASIS time points, and discharges throughout each patient’s episode of care.
• Completes point-of-care documentation of skilled services, adverse events, and relevant communications in medical records.
• Participates in Quality Assurance and Performance Improvement initiatives to help the agency meet its goals.
• Meets agency productivity standards and maintains accurate time logs and mileage records according to agency policies, including start- and end-of-day commuting procedures.
• Communicates with the scheduler daily and plans efficient travel routes to maximize continuity of care.
• Supervises Occupational Therapy Assistants, conducts required supervisory visits, and supervises CNAs when other disciplines are no longer involved in the care plan.
• Maintains occupational therapy licensure and continuing education requirements, adheres to ethical standards established by the licensing board and Partners in Home Care policies, participates in scheduled agency educational opportunities, and performs other duties as assigned by the supervisor.
