Home Health Physical Therapist (Traveler) – Massachusetts

Position Summary
The Physical Therapist provides skilled physical therapy services to patients in a home health setting. This role focuses on delivering direct patient care, collaborating with interdisciplinary teams, and contributing to individualized care plans to support patient recovery and independence.

Key Responsibilities

Provide in-home physical therapy services to assigned patients
Develop and implement comprehensive plans of care in collaboration with healthcare team members
Perform patient assessments and document outcomes accurately (including OASIS documentation)
Recommend and train patients in the use of rehabilitation equipment, including:
Splints
Orthoses
Prostheses
Wheelchairs
ADL equipment
Therapeutic modalities
Educate patients and caregivers on treatment plans and home safety
Work independently with minimal supervision
Required Qualifications
Minimum 2+ years of acute care (hospital-based) and home health experience
Home Health Certification required
Proficiency in OASIS documentation (NON-NEGOTIABLE)
Active Massachusetts Physical Therapist License
BLS certification (American Heart Association)
Valid Driver’s License
Must have own reliable vehicle
Ability to work independently
Preferred Qualifications
Additional acute care experience

1 supervisory reference (within last 2 years)
Copy of MA PT License
Copy of AHA BLS Certification
Traveler’s permanent address included in profile
All Requested Time Off (RTO) must be submitted upfront
