Description

📍 Pittsfield, MA | 🗓️ 13 Weeks | ⏱️ 40 hrs/week | ☀️ Days (8:00 AM – 4:30 PM)

Seeking an experienced Home Health Physical Therapist to provide in-home rehabilitation services for patients across the community. Candidates must have prior home health experience, be proficient with OASIS documentation, and be able to work independently.

Responsibilities:

Evaluate patients and develop individualized physical therapy treatment plans
Provide skilled in-home physical therapy services to improve mobility and functional independence
Assess the need for rehabilitation equipment, including splints, orthoses, prostheses, wheelchairs, and ADL equipment
Perform and document OASIS assessments accurately
Collaborate with physicians and the interdisciplinary home health team
Educate patients and caregivers on treatment plans, safety, and home exercise programs
Maintain timely and accurate clinical documentation

Requirements:

Active Massachusetts Physical Therapist License
Current AHA BLS certification
Minimum 2 years of acute care and home health experience
Home Health certification required
OASIS proficiency is required (non-negotiable)
Valid driver's license and reliable personal vehicle
Ability to work independently with minimal supervision
Acute care experience preferred

Assignment Details:

2 openings
13-week assignment
40 hours per week
Day shift: 8:00 AM – 4:30 PM
Rotating weekends and holiday coverage as needed
No on-call
Home Health assignment
Meditech EMR
Candidates must reside more than 50 miles from the facility
Preferred commute of 45 minutes or less from the Pittsfield office (service area)
