📍 Gloversville, NY | 🗓️ 12 Weeks | ⏱️ 40 hrs/week | ☀️ Days (8:00 AM – 4:30 PM)

Seeking an experienced Physical Therapist for a Skilled Nursing Facility. The ideal candidate will have prior SNF experience, strong rehabilitation skills, and the ability to work collaboratively with an interdisciplinary care team to help residents achieve their highest level of function.

Responsibilities:

Evaluate and develop individualized physical therapy treatment plans
Provide therapeutic interventions to improve mobility, strength, balance, and functional independence
Monitor patient progress and modify treatment plans as needed
Educate patients, families, and caregivers on rehabilitation goals and home exercise programs
Collaborate with physicians, nursing staff, and other rehabilitation professionals
Maintain accurate and timely documentation of patient care
Float to other areas as needed based on facility requirements

Requirements:

Active Physical Therapist License
Prior Skilled Nursing Facility (SNF) experience required
Previous travel experience required
Strong assessment, treatment planning, and patient education skills

Assignment Details:

12-week assignment
40 hours per week
Day shift: Monday – Friday, 8:00 AM – 4:30 PM
No weekends
No holidays
No on-call
Floating required
Skilled Nursing Facility setting
