Licensed Practical Nurse (LPN) - Home Health
Pay Range: $31.69 – $37.29 / hour
Job Summary The Licensed Practical Nurse (LPN) performs selected nursing care, personal care, support, and training for home health patients, family members, and caregivers based on an established plan of care. Serving sub-acute, convalescent, and chronic patients across all age groups, the LPN delivers intermittent nursing services in residential settings, personal care homes, and assisted living facilities.
Responsibilities
- Provides quality, safe delivery of home health services under the direction of a Registered Nurse (RN), including wound care requiring aseptic technique, catheter care, injections, bowel programs, colostomy care, and medication management.
- Administers treatments and medications in strict compliance with physician orders.
- Assesses patient home environments, provides health education and instruction, and communicates changes in patient condition to team members and physicians.
- Documents care, observations, and clinical activities in the electronic medical record (EMR) system in a timely manner.
- Performs therapeutic exercise regimes, range of motion activities, and personal care services as directed.
- Performs blood draws and collects diagnostic specimens as needed.
- Participates in rotating on-call coverage for after-hours, weekend, and holiday patient care needs.
Requirements & Certifications
- Completion of an accredited Licensed Practical Nursing program.
- Active State of Montana or Compact LPN license.
- Current American Heart Association BLS certification.
- Valid Driver’s License, reliable vehicle, and active auto insurance.
