### Required Experience, Certifications, Licensure, and Skills - **Licensure/Certifications**: - ARRT certification necessary - BLS (Basic Life Support) required - ACLS (Advanced Cardiovascular Life Support) preferred but not required - **Scheduling Requirements**: - On-call obligation: 4-5 nights per pay period, including 1 weekend a month - 30-minute call response time - **Health and Safety Compliance**: - Fit Test - Annual TB Questionnaire - 10 Panel Drug Screen - COVID-19 Vaccine - Influenza Vaccine - MMR (Measles, Mumps, Rubella) - Hepatitis B - 2 Step PPD OR IGRA (Quantiferon/Tspot) - Varicella - Tdap (Tetanus, Diphtheria, Pertussis) - Physical/Health Statement - **Professional Compliance**: - License/Certificate Verification - Code of Conduct Form - I9 - Confidentiality and Security Agreement - Integrity and Audit Services Form - Joint Commission Testing - Profile for Onboarding - Conflict Of Interest Disclosure Form - Criminal Background Check - Education Verification - System Access Form - Skills Checklist - **Optional Documentation** (not required for submission but needed for onboarding): - Color Vision Test - Latex Allergy Questionnaire
