- BLS, ARRT, WI Radiographer License - Prior IR experience including Neuro IR experience highly preferred - EPIC experience - May need to float onsite within skill set or to sites within 40 miles if needed Credentials and Personal Health History for onboarding include: - License/Certificate Verification - Ascension Core Competencies - Ascension Medical Imaging Contract Worker Onboarding (and Education Attestation) - MMR, Fit Test, Hepatitis B, Influenza Vaccine, Health Statement, 2 Step PPD OR IGRA (Quantiferon/Tspot), Varicella, Tdap, 12 Panel Drug Screen, Color Vision Professional Requirements for onboarding include: - Driver's License/State ID - Ascension Profile - Ascension 2 Professional References - Ascension Criminal Background Check Results - Ascension EPLS - Ascension Skills Checklist - Ascension Specialty Exam - Ascension Education Verification - Ascension Confirmation of Assignment - Ascension Employment Verification - Ascension Badge Photo Optional Requirements: - Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire - Hiring Manager Evaluation - Ascension Name Change Document
