**Certifications Required:** - Basic Life Support (BLS) Certification - Advanced Cardiac Life Support (ACLS) Certification - National Institutes of Health Stroke Scale (NIHSS) - Successful completion of a proctored rhythm test **Credentials and Licensure Requirements:** - Nursys License/Certificate Verification for submission - Driver's License/State ID **Healthcare Requirements:** - Ascension Color Vision, Core Competencies, EKG Exam, Unit Specific Checklist (optional), and 12 Panel Drug Screen for onboarding - MMR, Hepatitis B, Influenza Vaccine, 2 Step PPD OR IGRA (Quantiferon/Tspot), Varicella, Tdap, Health Statement for onboarding - Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire (optional) **Professional Requirements:** - Ascension Onboarding Requirements and Nursing Contract Worker Onboarding for onboarding - Ascension Nova StatStrip, Cerner Test-Out, Education Attestation for onboarding - Education Verification, Confirmation of Assignment, Employment Verification for onboarding - Profile, 2 Professional References, Criminal Background Check Results, EPLS, Skills Checklist for onboarding **Optional Professional Components:** - Hiring Manager Evaluation, Name Change Document, Badge Photo for submission - Ascension Professional References (for submission) - Ascension Badge Photo for submission **Additional Notes:** - Professional references and Ascension Professional Profile are for submission - All health-related documentation needs to be verified during onboarding process
