The job description specifies the following requirements: 1. **Credentials and Licensure:** - Nursys License/Certificate Verification for Submission. 2. **Checklist and Testing:** - Ascension Pharmacology Exam, Medical Surgical Exam, Core Competencies, EKG Exam, and Unit Specific Checklist required for onboarding (some optional). 3. **Professional Requirements:** - Ascension Onboarding Requirements, Nursing Contract Worker Onboarding, Profile, Professional References, Criminal Background Check Results, EPLS, Education Verification, Confirmation of Assignment, and Employment Verification required for onboarding. - Driver's License/State ID and Ascension 2 Professional References for Submission. 4. **Personal Health History:** - MMR, Hepatitis B, Influenza Vaccine, Fit Test, Health Statement, 2 Step PPD OR IGRA (Quantiferon/Tspot), Varicella, Tdap, 12 Panel Drug Screen for onboarding. - Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire optional. 5. **Certifications:** - Nursys, License/Certificate Verification. 6. **Additional Details:** - An Ascension Badge Photo is required for onboarding. 7. **Optional Submissions:** - Hiring Manager Evaluation and Name Change Document are optional for onboarding. This role involves working on an Ortho/Neuro unit with a 1:6-8 patient ratio, and requires the pertinent certifications, licensures, health records, and professional verifications for onboarding.
