This job requires a number of health and professional documentation for onboarding purposes, although these are not required for submission. Personal health history items include vaccinations and tests such as COVID-19 Vaccine, MMR, Hepatitis B, Influenza Vaccine, 2 Step PPD OR IGRA, Varicella, Physical/Health Statement, Tdap, and Fit Test. Professional documents needed for onboarding include a Trinity PowerPoint Genesis file, Trinity Conf. of Patient & Pty. Info. & System Security Agreement, Trinity Conflict Of Interest Disclosure Form, Trinity Criminal Background Check, Trinity Education Verification, Trinity Integrity and Audit Services Form, among others. Additionally, there is a requirement for a Trinity Skills Checklist and Trinity Specialty Specific Competency Testing. A License/Certificate Verification is required for both certifications and licenses as part of the process.
