- OR experience: 2 years experience in a busy OR department is required. - EPIC experience is mandatory. - Certification: Nursys - Licenses: Nursys - Required Documentation and Health Screenings: - Driver's License/State ID - Prime Drug Screening (minimum 10-panel test) - Prime Color-blind test - Prime Agency Attestation - Prime SAM/EPLS - Prime Education Verification - COVID-19 Vaccine - MMR - Fit Test - Hepatitis B - Influenza Vaccine - TB Test - Varicella - Tdap - Annual TB Questionnaire - Social Security Card - Physical/Health Statement - Prime Core Competencies Exam - Prime Criminal Background Check - Prime Badge Photo - Prime Form I-9 (Employment eligibility verification) - Prime Job Description - Essentials Onboarding Packet - Onboarding Education Attestation - Preferred Documentation and Health Screenings: - Positive PPD- Negative Chest X-Ray (optional) - Name Verification Affidavit (optional) - Prime COA (optional) - On Call Requirements: One night every 2 weeks, one weekend every 6 weeks. - Work Schedule: Five 8-hour shifts from M-F, work day hours from 0645-15:15, may extend dependent on case load. - Professional References: Prime Reference #1 and Reference #2 required for submission.
