**Certifications and Licenses Required:** - BLS (Basic Life Support) - ACLS (Advanced Cardiovascular Life Support) - NIH (National Institutes of Health Stroke Scale) - Nursys License - License/Certificate Verification **Experience Required:** - 2 years nursing experience - Experience with EPIC - Prior travel experience **Position Requirements:** - Float Position for CSM-Ozaukee: Must float onsite to where greatest need is, including departments such as Acuity Adaptable Unit (AAU), Med/Surg, Inpatient Rehab, and ED. - May need to float to sites within 40 miles if needed. **Professional and Personal Health Requirements for Onboarding:** - Ascension Badge Photo - Ascension Specialty Exam - Ascension Core Competencies - Ascension EKG Exam - Ascension Nova StatStrip - Ascension Epic Test-Out - MMR (Measles, Mumps, Rubella) - Fit Test - Hepatitis B - Health Statement - Varicella - Tdap (Tetanus, Diphtheria, Pertussis) - 2 Step PPD OR IGRA (Quantiferon/Tspot) - Ascension Skills Checklist - Ascension Criminal Background Check Results - Ascension EPLS - Ascension Education Verification - Ascension Confirmation of Assignment - Ascension Employment Verification **Optional or Additional Requirements:** - Influenza Vaccine (Optional) - Positive PPD-Negative Chest X-Ray and Annual TB Questionnaire (Optional) - Driver's License/State ID (Required for Submission) - Ascension Profile (Required for Submission) - Ascension 2 Professional References (Required for Submission) - Hiring Manager Evaluation (Optional) - Ascension Name Change Document (Optional)
