The job description outlines a range of required credentials and verifications needed for onboarding, not for the submission process. These include: - License: Certificate verification required for onboarding. - Various personal health history records, including fit test, drug screen, and vaccinations (COVID-19, MMR, Hepatitis B, Varicella, Tdap), as well as a physical/health statement. - Professional documentation and agreements, such as Trinity Code of Conduct Form, Integrity and Audit Services Form, and Confidentiality and System Security Agreement. - Criminal background check, education verification, and other professional standards compliance forms like Trinity GSA/OIG and Joint Commission Testing. - Specialty-specific competency testing and skills checklist. - Optional Influenza Vaccine for health-related history during onboarding. - Professional forms and disclosures, including Conflict of Interest, Child and Adult Dependent Abuse Training, and others related to system access and non-employee status. It is crucial that all these verifications and health records are completed prior to onboarding, as they are mandatory.
