The job description contains requirements for license/certificate verification, and several mandatory health and professional documentation checks, including: - License/Certificate Verification - Personal Health History, such as Fit Test, COVID-19 Vaccine, MMR, Hepatitis B, Varicella, Physical/Health Statement, 2 Step PPD or IGRA, and Tdap. - Professional documentation like I9, Code of Conduct Form, Conf. of Patient & Pty. Info. & System Security Agreement, Integrity and Audit Services Form, Non Employee Upload Form, Profile, Conflict Of Interest Disclosure Form, Criminal Background Check, Education Verification, EPLS, GSA/OIG, Joint Commission Testing, NSO, System Access Form, and Child and Adult Dependent Abuse Training. - Specialty specific competency testing and skills checklist. These documents and certifications are primarily required for onboarding rather than submission, with only the Trinity Profile needed for submission. Optional requirements include the Influenza Vaccine.
