**Licensure / Certification / Registration:** - BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. Accepted by American Heart Association or American Red Cross. - Registered Nurse obtained prior to hire date or job transfer date required. - Advanced Life Support preferred. Accepted by American Heart Association or American Red Cross. **Education:** - Diploma from an accredited school/college of nursing OR required professional licensure at time of hire. **Experience:** - Experience with multiple modalities (Trauma, Interventional Radiology, Cardiovascular, Vascular) is preferred. - Monitored experience with Merge Hemo is required. **Additional Requirements:** - Must be able to respond in 30 minutes during call shift. **Credentials:** - Nursys License/Certificate Verification **Professional Documentation:** - Ascension 2 Professional References - Driver's License/State ID - Ascension Profile **Health History Requirements:** - 2 Step PPD OR IGRA (Quantiferon/Tspot) - MMR, Hepatitis B, Varicella, Tdap vaccinations - Fit Test - Health Statement **Testing/Qualifications:** - Ascension Specialty Exam - Ascension EKG Exam - Ascension Color Vision Test - Ascension Core Competencies - Ascension Cerner Test-Out - Ascension Skills Checklist **Professional Onboarding:** - Ascension Onboarding Requirements Education Attestation - Ascension Nursing Contract Worker Onboarding - Ascension Nova StatStrip and educational verifications - Ascension Criminal Background Check Results - Ascension EPLS - Ascension Employment and Education Verification - Ascension Confirmation of Assignment **Optional Submissions (Onboarding):** - Ascension Badge Photo - Ascension Influenza Vaccine records - Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire - Ascension Name Change Document - Hiring Manager Evaluation
