Two (2) References
Two (2) professional references from work history occurring within the past three (3) years. One must be from a supervisor (charge nurses are considered supervisors). Reference must include the full name (first & last name) of the person providing reference. The name of the facility and the dates of employment must match the resume. Reference must include a breakdown of skills and strengths with a rating score. Letters of recommendation will not be accepted.

Verified within 30 days of submission. Nursys acceptable.

Competency Exam
Completed within one (1) year prior to start date for the specialty corresponding to the CP’s Assignment; required annually. An 80% passing score is required.
Driver's License or Government ID

Drug Screen
10-panel drug screen collected within sixty (60) days prior to start date; required annually.
Hepatitis B
Shown by proof of vaccination series, positive surface antibody titer, or declination.
Influenza
For Assignments occurring between 10/1-3/31. Declination is permitted with the understanding that CP will wear a mask at all times while at the facility, with the exception of while eating.
Joint Commission - Core I, II, and III Modules
Completed within one (1) year of start date; required annually. An 80% passing score is required.
MMR
Shown by proof of two (2) MMR vaccinations or positive IGG titers. If low or equivocal titer result, traveler will need to have an additional 2 doses of MMR administered.
OIG (Office of Inspector General)
Completed within thirty (30) days of start, to include all aliases; required annually and at time of extension
Physical
Completed within one (1) year prior to start date; required annually. Must be signed by MD, DO, DC, NP, or PA.
SAM (System for Award Management)
Completed within thirty (30) days of start, to include all aliases; required annually and at time of extension
TB
Documentation of TB skin test, TB QuantiFERON, or T-Spot completed within six (6) months prior to start date and annually thereafter. Chest X-Ray should only be completed for past positive PPD cases as a result of TB exposure. Chest X-Ray result must indicate that the Chest X-Ray was completed to rule out positive TB results. The Chest X-Ray results are good for 5 years. If providing Chest X-Ray, TB questionnaire is required at time of placement and annually thereafter.
Tdap
Administered within ten (10) years prior to start date.
Varicella
Shown by proof of two (2) vaccinations, positive IGG titer, or provider verified history.
