Patient Care Coordinator RN Job Details

• Location: Clackamas, OR 97015 (Portland metro area)
• Assignment Type: Travel RN position
• Healthcare Setting: Outpatient, Staff Lead
• Start Date: September 26, 2025
• End Date: December 20, 2025
• Duration: 12-week nursing contract
• Shift: 8-hour day shifts, 08:00 AM – 05:00 PM
• Guaranteed Hours: 40 hours per week
• Weekend/Holiday Coverage: May be required (Sunday–Saturday, including holidays)
• Estimated Weekly Salary: $2,560 - $2,750

Registered Nurse Qualifications

• Active RN license in Oregon, Washington, or compact state (required for travel nursing jobs in Oregon)
• Minimum 1 year of experience as a Patient Care Coordinator RN or Case Manager
• BLS certification (American Heart Association)
• Proficiency with EPIC electronic health records (EHR) system
• Knowledge of Medicaid and Medicare processes
• Experience in outpatient nursing or staff lead roles preferred
• Previous employment with large healthcare systems is a plus

Patient Care Coordinator RN Responsibilities

• Coordinate patient care activities and serve as a clinical resource for nursing staff in the outpatient setting
• Facilitate communication between patients, families, and the healthcare team
• Ensure compliance with individualized care plans, documentation standards, and regulatory requirements
• Utilize EPIC for accurate and timely charting and patient records management
• Assist patients with Medicaid and Medicare navigation and resource access
• Support scheduling, patient flow, and process improvement initiatives in the clinic
• Provide leadership, mentorship, and clinical support to nursing and allied health staff
