Summary
The PRN Case Manager RN provides professional nursing expertise in care coordination, discharge planning, and resource management to ensure high-quality, cost-effective patient care across the continuum. This role applies clinical knowledge, critical thinking, and utilization review principles to promote timely, safe, and appropriate transitions of care. The Case Manager serves as a key member of the multidisciplinary team, advocating for patient needs, collaborating with providers, and ensuring optimal outcomes through effective planning and communication.
Requirements
Education:
Graduate of an accredited school of nursing.
Associates Degree required; Bachelors Degree in Nursing (BSN) preferred or actively pursuing.
Experience:
Minimum of 3 years of recent clinical nursing experience required.
Previous experience in case management, discharge planning, utilization review, or care coordination strongly preferred.
Familiarity with community resources and post-acute care services beneficial.
Licensure:
Current and active Registered Nurse (RN) license in the State of Florida required.
Certifications:
Basic Life Support (BLS) American Heart Association required.
Case Management certification (ACM, CCM, NACCM, or CMCN) preferred.
Skills and Abilities:
Strong assessment, organizational, and critical-thinking skills.
Excellent communication and interpersonal abilities to collaborate with patients, families, and interdisciplinary teams.
Knowledge of medical necessity criteria, insurance authorization processes, and hospital discharge planning standards.
Ability to work independently, manage multiple priorities, and adapt to changing patient and departmental needs in a PRN capacity.
