Shift: Monday - Friday 8:00:00 AM to 4:30:00 PM
Minimum to Midpoint Pay Rate:$51.54 - $67.01 / hour

This pharmacist will work as part of the multidisciplinary care team to ensure optimal medication therapy management for patients with advanced heart failure.
Key Responsibilities Include:
- Collaborative Practice Agreement (CPA)based medication management, including initiation, titration, and monitoring of therapy
- Heart failure guideline-directed medical therapy (GDMT) optimization
- Medication reconciliation and transitions-of-care support
- Patient education and counseling on medications, adherence, and side-effect management
- Monitoring labs, vitals, and clinical response to therapy
- Participation in clinic visits, care coordination, and documentation within the medical record
- Comprehensive medication therapy management (MTM)
- Direct collaboration with specialty pharmacy services to streamline management of high-cost regimens to ensure therapy continuity
- Prior authorization management and medication access support
Professional Requirements:
- Must obtain and maintain an active Consultant Pharmacist license within 3-6 months of hire.
- In alignment with the health systems commitment to cardiovascular excellence, the pharmacist on this role must pursue and obtain the HFSA Heart Failure Certification (HF-Cert) within 3 years of hire.
Requirements
Education: PharmD required or BS Pharmacy degree with equivalent and acceptable experience from a College of Pharmacy accredited by American Association of Colleges of Pharmacy (AACP).
Experience: Five years of professionally responsible pharmacist experience required for Bachelors degree holders. One year of recent inpatient hospital experience as staff pharmacist or pharmacy intern in a general medical/surgical hospital preferred for PharmD candidates.
Certification:BLS (American Heart Association/Basic Life Support) required (must be obtained within first 90 days of hire). ACLS (AHA) and/or PALS (AHA), depending on practice site.
License: State of Florida Department of Professional Regulation, Registered Pharmacist Licensure.
Other: Strong communication and interpersonal skills, data entry skills, analytical and basic math skills required.
