Description
Job Opportunity: Physical Therapist - Outpatient
Location: Kaplan, Louisiana
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Description
- Primarily outpatient setting with occasional inpatient flexibility.
- Typical patient visits: approximately 10 per day.
- Shift hours: 7:30 AM – 4:00 PM.
- Dress code: scrubs in maroon/wine/burgundy colors.
- Upon assignment start, you may be required to join an Emergency Team and stay onsite during emergency events or be part of a relief team as directed by your unit manager.
Required Qualifications
- Valid Physical Therapist license in Louisiana.
- Minimum 1 year of Physical Therapist experience within the last year.
- Basic Life Support (BLS) certification issued by the American Heart Association.
- Experience with Epic electronic health record system.
Additional Requirements
- Valid Driver’s License or government-issued photo ID.
- Background checks including 7-year criminal history, EPLS/GSA/SAM, HHS/OIG, and sex offender search.
- Education verification of highest degree.
- Physical exam within 12 months indicating good health.
- Negative 10-panel drug screen within 60 days of start.
- Immunizations and health documentation including:
- Tetanus/Diphtheria/Tdap (if applicable)
- Negative TB test or chest X-ray within 12 months
- Measles, Mumps, Rubella, Hepatitis B, Varicella immunity or vaccination documentation
- Seasonal flu vaccination or approved exemption during flu season
- Fit mask testing (N95) within one year of start; onsite fit testing available if needed.
- Photo for badge (passport-style, recent, plain background).
- Skills checklist and employment application with 7 years of employment history and explanation of gaps.
Notes
- Candidates must be comfortable with emergency team assignments during events.
- Certifications such as ACLS, PALS, or others are not required for this role unless specified by unit.
- CPI (Crisis Prevention Institute) certification and other specialty-specific certifications are not required for this position.
