Description

Physical Therapist - Outpatient (Travel/Contract)

Location: Lakeport, California
Contract Length: 13 weeks


Job Description

  • Provide physical therapy services primarily in an outpatient setting.
  • Plan and implement individualized treatment programs based on physical therapy principles and practices.
  • Serve as a resource and provide structured educational opportunities within the facility in your area of specialization.
  • Work schedule: Monday to Friday, 8:00 AM to 5:00 PM (5 days x 8 hours), no floating or on-call duties.

Required Qualifications

  • Valid Physical Therapist license in California.
  • Minimum 2 years of physical therapy experience, including experience within the last year.
  • Bachelor’s degree (B.S. or B.A.), Master’s degree (M.A. or M.S.), or Doctor of Physical Therapy (D.P.T.) degree.
  • Basic Life Support (BLS) certification.

Background and Compliance Requirements

  • 7-year background check required within 6 months of start, including Social Security trace with all aliases, county searches, nationwide sex offender search, education verification, and FACIS Level 3 screening.
  • Annual updates required for EPLS/GSA/SAM, HHS/OIG, and FACIS screenings.
  • Official transcripts or diplomas may be required if education verification is unsuccessful.
  • Negative 9-panel drug screen within 6 months of start (excluding marijuana/cannabinoids due to California law).
  • Physical exam within 1 year of start, including clearance for work and any necessary accommodations.
  • Fit test for approved masks required within 1 year of start and renewed annually (exceptions apply).
  • Immunizations and titers required:
    • Tetanus/Diphtheria/Tdap (within 10 years)
    • Negative TB test or chest x-ray (annual renewal required)
    • Measles, Mumps, Rubella (MMR) immunity or vaccination
    • Hepatitis B series or signed declination
    • Varicella immunity or vaccination
    • Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
  • Respiratory safety clearance required prior to start and renewed annually (exceptions apply).
  • Color vision exam required if applicable (in-person exam only).

Documentation and Orientation

  • Must review and sign facility-specific documents prior to start, including:
    • Job description
    • Standards of Business Conduct
    • Code of Conduct and Overpayment Refund Policy (for current and future years as applicable)
    • General Regulatory Orientation for Contracted Workers
    • Vendor Training (required for contracts signed on or after 5/31/24 and 7/7/25)
  • Complete TB screening questionnaire within 90 days of start, signed by candidate and healthcare provider (MD, PA, NP/APN, DO, or RN).
  • Submit a clear, passport-style photo for badge (if applicable).
  • Employment application must include a full 7 years of work history with no gaps, including city and state of each location.
  • Employment verification required for up to 7 years, with explanations for any gaps over 30 days.
