Description
Physical Therapist - Outpatient
Location: Pleasant Prairie, Wisconsin
Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Start Dates: June 6, June 15, June 29, or ASAP
- Shift: Day shift, 4 days per week, 10 hours per day (7:00 AM - 5:00 PM)
- Experience Required: Minimum 1 year of experience in outpatient physical therapy with neuro experience
- First Time Travelers Accepted: Yes
- Locals Accepted: Yes (at a lower rate)
- Pending License Accepted: Yes
Required Certifications
- BLS (Basic Life Support): ARC or AHA acceptable; Provider, Instructor, and RQI accepted
- ACLS, PALS, NRP, NIH Stroke Certification, and other certifications may be required depending on specialty or department but are not mandatory for this role
Required Qualifications
- Valid Physical Therapist license in Wisconsin
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in an outpatient setting
- BLS certification
Background and Compliance Requirements
- 7-year background check including SS trace, county searches for all locations lived/worked/schooled, nationwide sex offender check (must be updated annually)
- State-specific caregiver screening for Wisconsin
- Photo for badge (passport-style, shoulders up, no filters)
- Confidentiality statement signed prior to start
- Compliance checklist to be completed by agency representative
Health and Safety Requirements
- Physical exam within 1 year of start
- Negative QGold TB test within 1 year (TSpot/PPD not accepted)
- Immunization or positive titer proof for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (titer, series, or declination)
- Negative 10-panel drug screen with expanded opiates within 14 days before start (instant tests not accepted)
- Seasonal flu vaccination or approved medical/religious exemption required during flu season (October to March)
Additional Requirements
- Two professional references verified within 2 years of start
- Annual skills checklist as per agency standards
Notes
- Prior travel experience is not required
- This position accepts candidates with a pending license
- Flu vaccine exemptions require completion of facility-specific exemption forms at orientation
