Job Overview

Required Qualifications
- Valid RN license in Indiana
- Med Surg (1.5 years of experience)
- Med Surg experience within the last year
- BLS - Basic Life Support issued by American Heart Association
- PALS - Pediatric Advanced Life Support issued by American Heart Association
- NIHSS
- Associate of Science in Nursing / Associate Degree in Nursing
- Driver's License
Required Pre-Start Documentation
- Valid Driver’s License or Passport
- Background check
- EPLS/GSA/SAM check
- HHS/OIG check
- Official transcript or official diploma
- Education verification
- Physical exam
- Fit test
- Negative TB test or chest X-ray
- TB questionnaire
- Measles immunity documentation
- Mumps immunity documentation
- Rubella immunity documentation
- Hepatitis B documentation
- Varicella immunity documentation
- Negative 10-panel drug screen
- Seasonal flu vaccination documentation or approved exemption
- Orientation safety manual acknowledgement
- Facility-specific orientation and compliance documents
