Occupational Therapist

Job Overview

Shift: Day, 5x8 hours, 8:00 AM–4:30 PM
Contract length: 13 weeks

Required Qualifications

Valid Occupational Therapist license in New Mexico
BLS - Basic Life Support issued by American Heart Association or American Red Cross

Required Health and Compliance Items

Background check required, including a 7-year criminal check and review of all locations lived/worked in the past 7 years
Physical exam within 12 months of start
Negative TB test or chest X-ray within 1 year of start
Tdap within the past 10 years
Measles, mumps, rubella, and varicella immunity documentation required
Hepatitis B documentation required
Drug screen required within 90 days of start
Flu vaccination or declination required during flu season
Fit testing completed onsite
Core competency exams required before start
Orientation and facility policy acknowledgements required before start

Certifications

BLS required
ACLS required as applicable
PALS required as applicable
NRP required as applicable
TNCC required as applicable

Additional Notes

75-mile radius restriction applies
Photo ID must be carried on person during hours of service
COVID declination form is available if declining vaccination
