Description

Rehabilitation Therapist

Location

  • Hilo, Hawaii

Employment Type

Contract Length

  • 13 weeks

Setting

  • Skilled Nursing Facility and/or Intermediate Care Facility

Required Qualifications

  • Valid Physical Therapist license in Hawaii
  • Physical Therapist (2 years of experience)
  • Physical Therapist experience within the last year
  • BLS

Schedule

  • Generally 5 days per week, likely including 1 weekend day
  • Day shift
  • May require floating to other units or facilities based on census and staffing needs
  • Must be available to work Hawaii State and Federal Holidays as needed

Responsibilities

  • Provide rehabilitation services
  • Perform evaluations, reassessments, treatments, and other clinical services
  • Develop, implement, or carry out individualized treatment plans
  • Provide evidence-based therapeutic interventions
  • Educate residents, families, and caregivers
  • Complete accurate, timely, and compliant documentation
  • Supervise therapy assistants when applicable

Training / Onboarding

  • Orientation to the rehabilitation department, EMR documentation, and facility workflows is provided

Start Requirement

  • Must arrive the Thursday before start for an employee health appointment on Friday
