Required Qualifications

Valid Physical Therapist license in Colorado
BLS
Driver's License

Preferred Experience

Home health experience
Familiarity with Medicare documentation and home health regulations
Experience working with geriatric and medically complex populations

Key Responsibilities

Perform physical therapy visits, including evaluations, start of care, reassessments, recertifications, and discharge visits
Develop and implement individualized plans of care
Provide gait training, balance training, therapeutic exercise, mobility training, and fall prevention
Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
Communicate patient progress or changes in condition to the interdisciplinary care team
Collaborate in the clinical oversight of PTAs when applicable and permitted under Colorado law and agency policy
Complete documentation accurately and on time
Participate in care coordination related to patient needs, visit completion, and regulatory requirements
