RN Case Manager

Job Overview

Location: Chillicothe, Ohio
Employment type: Contract
Schedule: Days, Monday–Friday, 7:30 a.m.–4:00 p.m.
Hours per week: 40
Contract length: 13 weeks
Caseload: About 20 patients per day

Role Summary

Assess discharge needs, social determinants of health, and discharge planning/referrals for acute medical and surgical patients.

Required Qualifications

Valid RN license in Ohio
Associate of Science in Nursing / Associate Degree in Nursing
Acute Care Case Management (3 years of experience)
Acute Care Case Management experience within the last year
BLS/BCLS
Must reside outside a 60-mile radius
