Home Health Physical Therapist

Overview

• Contract role in Lewistown, Montana
• 13-week assignment
• 36 hours per week, day shift

Required Qualifications

• Valid Physical Therapist license in Montana
• Physical Therapist experience within the last year
• 2 years of Physical Therapist experience
• BLS
• Driver's License

Role Summary

• Receive and review physician therapy orders for Home Health and Hospice patients
• Complete patient evaluations and create written treatment plans with short-term and long-term goals
• Provide physical therapy interventions such as therapeutic exercises, stretching, mobility training, balance activities, pain management techniques, and functional rehabilitation
• Assess home safety and recommend equipment, assistive devices, or environmental modifications
• Monitor patient progress and update treatment plans as needed
• Educate patients, caregivers, and family members on treatment plans, home exercise programs, safety measures, and expected outcomes
• Maintain clinical documentation, including evaluations, progress notes, discharge summaries, recertifications, resumptions of care, and discharge documentation
• Collaborate with physicians, nurses, therapists, and other healthcare professionals
• Travel to patient homes and other care settings to provide therapy services
