Description
Social Worker or RN Case Manager
Overview
- Travel/contract role in Baltimore, Maryland
- Day shift: 5x8, Monday-Friday, 8:00-16:30
- Contract length: 13 weeks
- Open to locals within 60 miles at a lower rate
Required Qualifications
- Valid Licensed Clinical Social Worker license in Maryland
- Licensed Clinical Social Worker with 2 years of experience
- Licensed Clinical Social Worker experience within the last year
- One completed travel assignment
- BLS - Basic Life Support issued by American Heart Association or American Red Cross
- Experience in an Acute Care setting
- Experience in an Acute Rehab setting
- Experience with discharge planning
- Experience with acute hospital case management
Additional Requirements
- Flu vaccine required
- Two positive supervisory references, including one from the most recent assignment, supporting at least 2 years of experience in the previous 3 years
- 7 years of work history
- Skills checklist required prior to start and updated annually
- Annual physical exam
- Negative TB test or chest X-ray
- TB questionnaire annually
- Drug screen within 90 days of start
- MMR immunity documentation
- Hepatitis B documentation
- Varicella immunity documentation
- N95 fit test within 9 months of start and updated annually
- Color blind testing
- Competency exam
- Background check
- EPLS/GSA/SAM check
- HHS/OIG check
- Education verification
- State license verification
- I9/E-Verify attestation
- Facility modules required by start date
