Physical Therapist Qualifications

Active Physical Therapist (PT) license in Pennsylvania
Minimum 1 year of experience as a Physical Therapist, preferably in long-term care, skilled nursing, or rehabilitation settings
Strong communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary healthcare team

Physical Therapist Responsibilities

Evaluate, plan, and implement individualized physical therapy treatment programs for long-term care residents
Collaborate with nursing staff, physicians, and other healthcare professionals to optimize patient outcomes
Document patient progress and maintain accurate clinical records in compliance with healthcare regulations
Educate and support residents and their families regarding therapy goals, exercises, and rehabilitation plans
Participate in interdisciplinary team meetings and contribute to patient care planning
