Job Description

The RN will provide comprehensive nursing care for residents, specifically in long-term care (LTC) facilities, while demonstrating the knowledge and skills necessary for the adult and geriatric patient groups. The position requires collaboration with interdisciplinary teams to ensure the effective planning and delivery of care. This is a part-time position requiring evening or night shifts based on the needs of the client.

Responsibilities

• Collect and assess data relevant to residents' physical and psychosocial conditions.
• Develop and implement care plans in partnership with residents and their families.
• Evaluate care plans and suggest revisions based on resident outcomes.
• Maintain collaborative relationships with other nursing staff, physicians, and ancillary staff.
• Participate in mentoring and orientation of new staff, contributing to a culture of continuous learning.
• Ensure compliance with safety, cost, and care delivery impact considerations.

Required Experience / Certifications / Licensure

• New York LPN Licensure
• Basic Life Support (BLS) certification (AHA or RQI only)
• Minimum of 1 year of experience as an RN in a long-term care setting
• Weekend and holiday shifts are required
