Job Description

The hospital-based skilled nursing facility provides supervised restorative care for older patients post-hospitalization or after an acute illness. Focusing on geriatric nursing, we aim to improve patients' health skills and functional abilities within the community.

Patients generally include those recovering from post-operative hips, knees, and shoulders, along with diagnoses such as pneumonia and congestive heart failure requiring extra rehabilitation.

Responsibilities

• Provide high-quality nursing care to patients in a skilled nursing setting.
• Collaborate with multidisciplinary teams to ensure optimal patient care.
• Perform assessments and develop patient care plans.
• Administer medications and treatments as necessary.
• Monitor patient progress and document observations in the electronic health record.

Required Experience / Certifications / Licensure

• Active West Virginia nursing license.
• BLS certification required.
• No false nails allowed per unit policy.
